We recognize the devastating impact of addiction in our community and, for many of us, in the lives of those we love and care for. Release of Information for Sharing Substance Use Records – Any information you provide to our Substance Use System of Care, including information provided to our Substance Use Access and Referral Helpline, is highly confidential and will not be shared with other individuals or entities without your permission. If you have provided information to any of our SUD System of Care providers and want them to share this information with any other individual or entity, including other SUD System of Care providers, please complete this Release of Information document and give it to the provider that holds the information you want shared with any individual or entity.
